Preparing for Your Visit
Pre-visit Questionnaires
Several pre-visit questionnaires will be emailed to you in order for us to get to know you better. Please fill them out with as much detail as you can and in a timely manner. This information will be an important part of your visit.
Medical Records
If possible, please bring any significant old medical records and the results of any prior imaging or lab studies. Please ask your prior physician or specialist to fax records to us at 805-563-3130.
Dementia Patient
If you have dementia or if memory is a concern, please make sure that a family member or a friend accompanies you to your visit. This is important to provide information on your memory as well as your past history.

COVID-19 Precautions
If you do choose to come to our office, we will take your health and safety very seriously. You will complete a quick COVID19 questionnaire before entering. We wear masks and ask that you do the same. Masks and hand sanitizer are provided if needed, and all surfaces, equipment, chairs, and exam tables are sanitized frequently.

Our Location
Our office is located at 515 E Micheltorena St, Suite C, Santa Barbara, CA 93103. It is on Micheltorena Street between Olive and Salsipuedes. There is a parking lot behind the building. There is handicapped parking available. We are on the first floor. Our office is shared by Drs. Bordofsky, Trautwein, and Rubio. Our chairs and exam tables were specifically chosen to make your visit as accessible and comfortable as possible.
